- Gender, Marital Status, Race, Ethnicity (Hispanic/Latino or not Hispanic Latino), and/or Protected Veteran status (pdf)
- Disability Status (pdf)
- Gender Designation Change: Students and employees may request a change of gender designation (pdf) within UMass Lowell databases or information systems
- Name Change: UMass Lowell will change the primary/legal name of employees with official documentation. Please see Massachusetts forms to change name. After an employee’s name is legally changed, the employee may notify UMass Lowell of the name change using the Name Change Form (pdf).
