    Use this form to officially update your HR Direct record for a legal name change or to update your email address to your preferred name.
    To update your preferred name in HR Direct only, you can follow the View & Maintain Personal Details job aid (pdf). Please note that this will not update your email address.
  • Sponsored Guest Form (docusign)
    Sponsored Guest Form is used only for a non-employee who needs access to UMass Lowell resources (Email, SIS, Finance, Parking, etc.).

New Employee Hire Packet

The new employee hire packet provides a method to electronically complete and securely submit your employment forms.

Form I-9

Benefited Faculty / Staff, Part-time (PT) Adjunct Faculty, and Temporary PT Staff New Hires will be provided a link via email that will direct them to verify their identity and eligibility to work in the United States, which is required under the Immigration Reform and Control Act of 1986.

  • Section 1 of the Form I-9 must be completed by the employee no later than thefirst day of employment.
  • Section 2 of Form I-9, you must present unexpired original documentation that shows proof of identity and employment authorization to work in the United States. Please refer to Instructions for Form I-9, Employment Eligibility Verification for more information. Please refer to Instructions for Form I-9, Employment Eligibility Verification (pdf), for more information. Failure to comply with these conditions of employment can result in the withdrawal of this employment offer.
    • When you complete Section 1 of Form I-9: please schedule your required remote I-9 Section 2 verification before or within three (3) business days of your start date by using our online scheduler. At this meeting, please have the proper unexpired documentation as described on the List of Acceptable Documents with you.

    • University Policy requires all employees to be familiar with the Intellectual Property Policy and Conflicts of Interest Outline.
  • Participation Agreement and Receipt of Policies (pdf)
    • Employees are required to sign a Participation Agreement and Receipt of Policies at initial hire, and whenever signing a renewed contract.
  • Mandatory Omnibus Budget Reconciliation Act (OBRA) Retirement Plan
    • As a part-time, seasonal, or temporary employee of the Commonwealth of Massachusetts at UMass Lowell, you are required to participate in the Mandatory OBRA Plan.
    • The Mandatory OBRA Plan serves as an alternative to Social Security, as authorized under the federal Omnibus Budget Reconciliation Act of 1990 (OBRA). Federal law requires that employees who are not eligible to participate in their employer's retirement program be covered by either Social Security or a qualifying alternative retirement plan. The Mandatory OBRA Plan satisfies these federal requirements.
    • Upon hire, you are automatically enrolled in the Mandatory OBRA Plan. A mandatory contribution of 7.5% of your eligible earnings will be deducted from your paycheck. No enrollment forms or additional action are required on your part. Once payroll deductions begin, your enrollment information will be submitted to Empower, the plan administrator. Empower will then mail you information about your retirement account, including instructions for accessing your account and designating your beneficiary.
