Teaching & Researching Assistants Payroll Forms:

• Terms of Employment for Grant Funded employees (doc)
  This form must be filled out and attached to the contract of all Grant-funded employees.
• TA/RA Payscale webpage
  TA/RA stipend levels table.
• Graduate Employee Organization (GEO) Union Membership (Dues and Fees) Form (DocuSign)
  All contracts for TA/RA must also be accompanied with this completed form.
• TA/RA/GA Performance Review (doc)
  This form should be filled out and kept in the Dean's office.
