Submit Key Request Online Form. One form per location and per key holder.

DO NOT submit a Service Request for key requests.

When an employee moves from their current location or leaves the university, their keys are required to be returned to either the Service Center, UCAPS or Human Resources.

Equipment Installation Requests

All equipment that is purchased, donated or relocated must be accompanied by an Equipment Installation Form. This form is required for the purpose of proper utility management/connections, safety controls and space allocation. To request an equipment Installation, please complete the Equipment Installation Request (note: initial login will require your permission for access, any questions email: FIS-Help@uml.edu) by signing into Microsoft using your UMass Lowell credentials.
