*On March 1,2017 the old Department ID Request Form will be retired. Any requests submitted using the old form after this date will be returned, requiring the requester to use the new DocUSign Department Request Form.

Save this document to your desktop and assign it a unique file name (i.e. the Department Name requested along with the date of the request, "Music Scholarship_021416").
