How to Register

To receive academic accommodations a student must be fully registered with our office.

Students who are requesting an accommodation(s) based on a documented disability must follow the steps outlined below to register with our office. Our specialists review documentation as a team, every Wednesday morning.

To register for Academic Accommodations, please have your required documentation ready to upload and follow the instructions below:

  1. Complete the Public Accommodation online Request Form. Please note that you will need your UMass Lowell student email and password to log into the system. This form must be filled out by the student, otherwise it will be rejected, delaying the application process. The student will be asked to fill out the form again.
  2. Upload your documentation to your completed Public Accommodation online Request Form. You can also view the types of documentation we accept.
  3. Once this process is complete, you will need to check your UMass Lowell email account for a notification of when you can book an Intake Appointment with our office. Intake appointments can be scheduled once both the Public Accommodation Request Form and the documentation have been reviewed by our team of specialists. 
  4. During this one-on-one meeting, our staff and student work together to establish an accommodation plan to help students to best access the course curriculum. Meetings are private and confidential. A student may alert DS staff if a family member would join us for a summary at the end of the meeting.

  • View and upload your documentation
