Take Our Survey
Thank you for joining our evaluation study.
Our goal is to learn how the Healthy Workplace materials perform in a range of organizations. Your feedback will help us refine the toolkit materials and design future training and outreach activities.
The study will consist of a 15-minute survey and one or more telephone interviews. An incentive of $20 will be offered for each completed survey or interview. Evaluation activities will be ongoing through May 2015.
