As Commonwealth of Massachusetts employees, disclosure of outside activities are required by the Massachusetts State Ethics Commission. UMass Lowell considers outside activities to be non-academic activities within your area of expertise that you take part in in association with individuals or entities outside the University. This includes any outside activities you take part in while on sabbatical.

UMass policies regarding outside activities and associated disclosures can be found at T96-047 - Policy on Faculty Consulting and Outside Activities (pdf), T96-039 - Policy On Conflicts Of Interest Relating To Intellectual Property And Commercial Ventures (pdf), and its PHS-NIH addendum (pdf).

There are separate forms for faculty and staff:

Additional guidance including a Summary of the Conflict of Interest Law may be found on the Mass.gov website.
