How to access the UMass Lowell Student Health Portal and upload health records.

Health Services Patient Portal

Please submit all documentation through the UMass Lowell Health Services Patient Portal. For electronic medical record security, we are unable to accept forms by fax or email.

Per Massachusetts regulations and University requirements, all full-time students, part-time health majors, and students living in on-campus housing are required to electronically complete self-reported health forms and upload an immunization record prior to coming to UMass Lowell. Compliance with the immunization requirements set forth by the Commonwealth of Massachusetts is mandatory for UMass Lowell students of all ages. Students must register for the Student Health Portal (instructions above) to complete these requirements.

ONLINE STUDENTS: Exclusively online students (for example: those without required on-campus symposia, intensives, seminars and more) are not required to submit health forms and immunization records if they do not participate in any activities or instruction on campus, regardless of major. Students who transition to any on-campus courses during their career at UMass Lowell will be required to submit immunizations and health history forms per policy.

Online Health Record

All newly enrolled students must complete the following mandatory electronic health forms located in the Forms section of their Health Services Patient Portal (PDF files below are previews only):

In addition, please download the Immunizations and Physical Examination forms below and bring them to your medical provider for completion and signature. Once signed by your provider, please upload a copy to the Patient Portal. Alternatively, we will accept a medical provider's standard form as long as it includes your complete immunization record and date of your last physical.

  • Immunizations (pdf): Upload your completed UMass Lowell Immunizations form to the Student Health Portal. Note: We do not accept records via fax, email, or hand delivery.
  • Physical Examination Form (pdf): Physical examination should be performed within the last two years.

The following immunizations are State and University required:

  • MMR series (measles, mumps, rubella)
  • Tdap on or after eleventh birthday (tetanus, diphtheria, pertussis)
  • Varicella series (Chicken Pox)
  • Hepatitis B series
  • Meningitis ACWY vaccine

Additional vaccines that are highly recommended, but not required for enrollment:

  • Two doses of the Meningococcal Group B vaccine
  • COVID-19 vaccine and boosters
  • Annual influenza vaccine
  • Booster dose of Tdap or TD every 10 years

Due Dates

All health records are required by August 1 for fall enrollment and January 1 for spring enrollment.

Failure to submit mandatory health records will result in a hold on your registration and possible removal from enrollment.
