Documentation Requirements

Documentation pertaining to your disability helps us work together to determine academic accommodations that will work best for you during your time here at UMass Lowell.

Please review the information below and choose what best describes your disability.

  1. I had an Individualized Education Program (IEP) or 504 in high school.

  2. I attended another college / university and received academic accommodations at that institution.

  3. I am a Military Veteran.

  4. I have a Psychological Disability and I am currently receiving treatment from a licensed professional.

  5. I have a Physical/Medical Disability and am currently receiving treatment from a licensed professional.

  6. I have ADD/ADHD and am currently receiving treatment from a licensed professional.

  7. I have a Learning Disability but did not receive services through an Individualized Program (IEP) or 504.

Descriptions on the different types of documentation requirements to register with our office have been listed below. Please choose the one that pertains to your disability and / or situation.

I had an Individualized Education Program (IEP) or 504 in high school. 

A copy of your IEP or 504, along with any neuropsychological assessments that were conducted as part of the plan can be uploaded to your Public Accommodations Request Form. A comprehensive assessment battery and resulting diagnostic report should include: Diagnostic interview, assessment of aptitude, academic achievement, information processing, and diagnosis. These documents cannot be older that 5 years. If your documentation is more than 5 years old, you may prefer to use on of our verification forms that apply to your disability. We offer the following: Psychological Verification Form (pdf), Medical Verification Form (pdf), ADD / ADHD Verification Form (pdf).

I attended another college/university and received academic accommodations at that institution. 

If you attended another college/university and received academic accommodations at that institution, a letter from the disability services office of that institution confirming the dates you attended and the accommodations you received as a student along with a copy of your latest documentation can be uploaded to your Public Accommodations Request Form. If you cannot obtain a letter from your previous institution, you may prefer to use one of our verification forms that applies to your disability. We offer the following: Psychological Verification Form (pdf), Medical Verification Form (pdf), ADD / ADHD Verification Form (pdf).

I am a Military Veteran. 

If you are a military veteran, A section of your discharged paperwork that gives the percentage of service connected disabilities according to the VA, may be uploaded to the Public Accommodations Request Form. Direction on how to obtain that paperwork from the VA can be found on our Veteran Documentation webpage. If you are unable to obtain information from the VA, you may prefer to use one of our verification forms which applies to your disability. We offer the following: Psychological Verification Form (pdf), Medical Verification Form (pdf), ADD / ADHD Verification Form (pdf).

I have a Psychological Disability and I am currently receiving treatment from a licensed professional. 

If you have a psychological disability and are currently receiving treatment from a licensed professional, you may download our Psychological Verification Form (pdf). You will need to complete the first section and your provider will need to fill out the form completely. Completed forms can be uploaded to your Public Accommodations Request Form.

I have a Physical/Medical Disability and am currently receiving treatment from a licensed professional. 

If you have a physical/medical disability and are currently receiving treatment from a licensed professional, you may download our Medical Verification Form (pdf). You will need to complete the first section and your provider will need to fill out the form completely. Completed forms can be uploaded to your Public Accommodations Request Form.

I have ADD/ADHD and am currently receiving treatment from a licensed professional. 

If you have been diagnosed with ADD/ADHD and are currently receiving treatment from a licensed professional, you may download our ADD / ADHD Verification Form (pdf). You will need to complete the first section and your provider will need to fill out the form completely. Completed forms can be uploaded to your Public Accommodations Request Form.

I have a Learning Disability but did not receive services through an Individualized Education Program (IEP) or 504. 

Students who have a learning disability but did not receive services through an Individualized Education Program (IEP) or 504 were likely diagnosed after they completed high school. In those cases, students may upload a copy of their neuropsychological assessment to their Public Accommodations Request Form. A comprehensive assessment battery and resulting diagnostics should include: Diagnostic Interview, Assessment of Aptitude, academic Achievement, Information Processing, and Diagnosis. These Assessments should not be older than 5 years.
