General Information about Documentation
Documentation should validate the need for accommodation based on an individual's current level of functioning. Please choose any of the following options below to provide documentation pertaining to your disability:
- Individualized Education Plan (IEP) or 504 Plan that you may have from High School and a copy of your latest neuropsychological evaluation that was conducted along with those documents. These documents cannot be older than 5 years. A comprehensive assessment battery and resulting diagnostic report should include: diagnostic interview, assessment of aptitude, academic achievement, information processing, and diagnosis.
- If you attended another college/university and received accommodations at the institution, a letter from their disability services office that states the dates you attended and the accommodations you received, along with the documentation you provided to receive accommodations.
- If you are a military veteran, you can send your discharge paperwork with the percentage of service connected disabilities you may have according to the VA.
- If you do not have access to any of the above, you may have your provider complete one of our verification forms. Please choose the verification form that best fits your diagnosis.
