Request for Name & Gender Designation
UMass Lowell will change your primary/legal name with official documentation.
Name Change: Students may indicated a preferred name in SiS. Class rosters will reflect the preferred name.
- Name Change: UMass Lowell will change the primary/lead name of students and employees with official documentation. Please see the Documentation for Massachusetts residents webpage.
- In addition to primary/legal name, students may use a “preferred name” which will be displayed on class rosters and in Blackboard LMS system.
- After an employee's name is legally changed, the employee may submit a name change form (pdf) to Human Resources and Equal Opportunity and Outreach.
Gender Designation Change: Students and employees may request a change of gender designation within UMass Lowell databases or information systems using the request for change to gender designation form. Please see the Request for Gender Designation form (pdf).
