Community Needs Assessment Survey on Opioid Use Disorder Care in Tewksbury
Are you at least 18 years old and living/working/volunteering in the City of Tewksbury? If so, you are invited to take this survey created by the Town of Tewksbury and UMass Lowell’s Center for Program Evaluation (CPE). Please share with anyone you know who might be interested in participating.
The purpose of this survey is to learn about the needs and barriers to care related to opioid use disorder (OUD). The survey takes about 12-15 minutes. As a token of our appreciation, you can enter a raffle for a chance to win a $25 Amazon gift card (four cards will be awarded) at the end of the survey. Please complete this survey by Oct. 5, 2025.
