Below are forms and applications pertinent to the Graduate Student Association (GSA).


Nomination Forms

  • GSA Club Registration Form (pdf) - for the 2022 - 2023 Academic Year - Deadline September 30 of each new year for seed funding eligibility. Clubs can still register after deadline and not receive seed funding.
  • Executive Committee (EXCOM) Nominations (pdf) - For the 2023 - 2024 Academic Year. Elections will be held in spring 2023.

GSA Club Awards

These three awards are presented at the Annual GSA Spring Awards Banquet. Nomination forms are due no later than 4:30 p.m. on March 31, 2023.

Reimbursement Forms

Please review the reimbursement guidelines prior to submitting a reimbursement form.

  • EXCOM Reimbursement Form (pdf)
  • Graduate Research Grant Award (GRGA) Purchase Order Form (use this form to have your materials ordered with a university issued purchase order. Include an official quote from the vendor. A separate form must be filled out for each vendor).

NOTE: To be eligible for Professional Development Awards (PDAs) applicants MUST submit completed, signed Travel Authorization and Liability waiver forms to the GSA advisor PRIOR to travel.

Fall 2022 Applications for the GRGA and PDA are CLOSED, applications are not longer accepted!

  • GSO Registration Form (pdf) - Due by September 30 of each year**, (** To be eligible for seed funding, registrations received after September 30, 2022 will not be eligible for seed funding, registrations received after the deadline are not eligible for seed funding)
  • GRGA Application Form (pdf) - Fall 2022 applications due by 5 p.m. on November 12, 2022, Spring 2022 applications due by 5 p.m. on April 7, 2023
  • PDA Application Form (pdf) - Fall 2022 applications are due by 5 p.m. on November 12, 2022, and for Spring 2023, April 7, 2023.
