How to Apply

ISSO Has a New CPT Application Process as of April 22, 2019!

The new application is completely paperless for international students and their CPT advisors. Because we have moved to this electronic application system, there are now three different e-form applications depending on the type of CPT you are requesting. Students should be sure to fill out the CORRECT CPT Application. Accidentally completing the wrong application, will result in denial. This may cause a delay in start dates, so please be careful when selecting the right e-form request for you.

SELECT ONE OF THE THREE E-FORM APPLICATIONS BASED ON THE CPT YOU ARE APPLYING FOR:

  1. One Credit CPT Course: CPT Application for students enrolling in a 1 credit CPT course through their department (generally this involves first receiving a job offer letter, and receiving permission from your advisor to work on a CPT in your academic department). Students will be required to upload the following documentation to the above CPT application e-form:
    • Job offer letter
    • Job description
    • Copy of unofficial transcript reflecting 1 credit CPT course registration
    • Form I-94
  2. Required Coursework or Degree Requirement: CPT Application for students registered for a service learning class, enrolled in a required practicum, participating in clinical rotations, or a required capstone project. Students will be required to upload the following documentation to the above CPT Application e-form:
    • Job offer letter
    • Copy of Unofficial transcript reflecting course
    • Form I-94
  3. Full-time Co-op Program Student: CPT Application for students accepted into the full-time Professional Co-op Program, or Graduate Master's Co-op Option in Engineering. Students will be required to upload the following documentation to the above CPT application e-form:
    • Job offer letter
    • Copy of unofficial transcript reflecting co-op registration
    • Form I-94

Please be aware of the Job Aids located on our forms webpage for CPT Approvers (Graduate Coordinators, Thesis Advisors, Undergraduate Advisors, etc).

All documentation must be submitted to the ISSO electronically via the appropriate above linked e-form. Students attempting to drop-off their documents in person or via email, will be referred back to upload their documents via the e-form.

You will receive an email notification within 5-7 business days beyond the date of submission, that your e-form has been reviewed. We appreciate your patience.
