Physical Accessibility Service Request

Use this form to submit a non-urgent physical accessibility service request to the Disability Office. DS will then share your request with facilities. If this is an urgent request (i.e. elevator broken, push button issue, weather related, etc.) please call the Facilities Management service center at 978-934-2601. For residential spaces, please submit a request through FixIt on the online housing portal.
