The following forms are available for you to communicate with the University about a change to name and/or gender. These forms will be used by UMass Lowell to keep the University’s records accurate.

  • Gender Designation Change: Students and employees may request a change of gender designation within UMass Lowell databases or information systems using this form: Request for Change to Gender Designation
  • Name Change: UMass Lowell will change the primary/legal name of employees with official documentation. Please see Massachusetts forms to change name. After an employee’s name is legally changed, the employee may notify UMass Lowell of the name change using this form: Name Change Form (pdf).
