Health Services Requirements

As a condition of admission to the University and subsequently, treatment at Health Services, all students are required to have the following forms on file:

• a complete, up-to-date medical history form (download this form)
• a complete, up-to-date physical examination form (download this form)
• evidence of a recent Mantoux test (if applicable), (download this form) and
• proof of up-to-date immunizations (download this form).

Immunization, medical history and physical exam forms are also available at the Student Health Services Office.

As of August 2007, the Massachusetts Department of Public Health has mandated that all new college students, who live in campus housing receive one of the Meningococcal vaccines or sign the Department of Public Health's waiver form.  Download this form or pick one up at Health Services. 

All full-time students who have met the Health Service requirements are eligible to use services. Please bring your student i.d. to your appointment.
