New Students
General Information about Documentation
Documentation should validate the need for accommodation based on an individual's current level of functioning. Most recent testing should have been done no earlier than late adolescence.
Records should be reflective of a student's current level of educational functioning. A school plan such as an Individualized Education Program (IEP) or a 504 Plan is insufficient documentation, though it may be included as part of a more comprehensive assessment.
A comprehensive assessment battery and resulting diagnostic report should include:
- (a) diagnostic interview,
- (b) assessment of aptitude,
- (c) academic achievement,
- (d) information processing, and
- (e) diagnosis.
Physical/Learning Disabilities
For physical and learning disabilities, appropriate documentation from one or more of the following sources it required:
- Clinical or Educational Psychologist
- School Psychologist
- Neuropsychologist
- Learning Disabilities Specialist
- Licensed medical practitioner
- Other appropriate licensed professional
For psychiatric disabilities, appropriate documentation must be provided by a licensed mental health professional and should include:

Date of diagnosis
DSM IV diagnosis
Brief description of the clinical data supporting the diagnosis and present level of functioning
Statement about how this disability may affect functioning in the academic setting
Newly accepted students who are requesting an accommodation based on a disability must interview with an admissions officer and the appropriate Student Disability Services personnel at the University in order to ensure delivery of needed accommodation(s). These interviews are scheduled through the Office of Student Disability Services.
Request for accommodation must be made by the student to the appropriate disability provider on a semester by semester basis.
