Documentation establishing the presence of a disability is the first step in legally obtaining higher education accommodations, and it helps us work together to determine academic accommodations that will work best for you during your time at UMass Lowell. Accommodations listed within the documentation will be considered as recommendations to help guide the interactive process.
All Documentation must meet the following initial guidelines. If it does not, your documentation will most likely not be approved and it will take longer to get registered with our office.
- Documentations mentioned below most be appropriate to your disability.
- Documentation must be on an official letterhead, fully completed and signed.
- Documentation must be dated within the past 5 years.
- Documentation must be on a non-editable format.
Types of documentation accepted for registration with our office are listed below. You only need to submit documentation from one of the options. Please refer to the Types of Documentation Accepted section below for guidance in selecting the option that best describes your disability circumstances.
- Disability Verification Form - This form is available to download from the Types of Documentation Accepted Section below.
- Individualized Education Program (IEP) or 504 Plan - An official copy, must be the complete report and must be signed.
- Neuropsychological Testing or Assessment - Should include: Diagnostic Interview, Assessment of aptitude, Academic Achievement, Information processing and Diagnosis
- Veteran Documentation - Please refer to the Veteran Documentation Requirement below.
- College Letter along with Supporting Documentation - College Letter must be on official letterhead, dated and signed, and must provide a copy of the supporting documentation.
Documentation may be uploaded to the Public Accommodations Request Online form (PARF). If a PARF has already been created, please email documentation to Disability Services at: disability@uml.edu.
