Co-op Contract Forms

Required components of the Co-op Contract are available below (in Microsoft Word). You can also obtain Co-op Contract forms from your co-op faculty advisor or the Career & Co-op Center in Southwick 328.

The Co-op Contract consists of four parts (or five parts for international students):

Co-op Learning Objectives Forms

A key component in any co-op experience is the development of learning objectives.  Following are forms to facilitate and develop learning objectives.

Co-op Evaluation Forms

At the midpoint and end of each co-op semester, evaluations of the student must be completed.  In addition, a site evaluation by the student is required.
