Change of Address

Students must complete a Student Information Change form from the Registrar's Office or notify the Registrar's Office in writing.

Change of Name

Students must complete the Student Information Change form from the Registrar's Office. A change of name also requires proper documentation, such as a driver's license, marriage certificate, etc.
