Transcript Requests

For students enrolled prior to January 2000, Transcript Request Forms are available online and in the Registrar's Office. Transcripts may also be requested in writing to the Registrar's Office at the address given below or faxed to (978) 934-4076. Written requests should include:

• dates of attendance
• student identification or social security number
• full name while in attendance
• full address of where you want transcript(s) forwarded
• signature
  

Students enrolled after January 2000 may print unofficial transcripts or request official transcripts through the ISIS Student Self-Service. UML does not provide transcripts to anyone with an outstanding obligation to the University.
