- Approval for Home Use of University Equipment. Complete when Property will be used off campus.
- Declaration of Usable Surplus Property. Complete when surplus is usable by other persons.
- Declaration of Unusable/Obsolete Surplus Property. Complete when surplus is unusable.
- Change of Equipment Location and/or Department. Complete when Property will be permanently moved from its current location.
- Return or Trade-In of Property. Complete when Property will be permanently moved from its current location.
