
STEP ONE: Click on the CAMIS Self Service link to take you to the LOG IN screen.
STEP TWO: After logging in, click on the button marked “Service Requests."
STEP THREE: If you wish to review the status of previously entered service requests, use the pull down arrow beside the “Search” box to select the time span in which you wish to review service requests. Then hit the “Go” button. You may review an individual service request by clicking on the service request number.
If you wish to create a new service request, click on the “Create Service Request” button.
STEP FOUR: Enter or confirm the required information: name, phone number and department. If desired you may add an “Alternate Requestor”. Otherwise, click on the “Next” button.
STEP FIVE: This will bring you to the “Select Service” screen. Click on New Web Request "Create your own request."
STEP SIX: This will bring you to the "Enter Service Details" screen. Please fill in the necessary information in the "Work Description" box. In the date blocks, YOU MUST FOLLOW THE DATE FORMAT SPECIFIED, mm/dd/yyyy. For example, September 5, 2007, would be entered as 09/05/2007. After entering the information, click on the “Next” button.
STEP SEVEN: Use the various buttons to select the “Site”, “Building”, “Floor” and “Room” for the service to be performed. Fields with an asterisk * are required fields. Not all floor and room numbers are recorded in the system at present. You may enter the floor and room in the “Work Description” box on the previous screen.
STEP EIGHT: You are now at the Review Screen. If everything is correct, click on the “Finish” button. If you have another problem to report, click on the “Go Back To Service Requests” button. If not, log out.
