Service Request Form

UMass Lowell uses a self-service request system called CAMIS to process and review work orders in a timely and efficient manner.  UMass Lowell employees may use their user name and password to logon to CAMIS below, but do not need to add "" after their user name.  Example:  ronald_smith, NOT

Urgent Service Requests

For urgent requests (i.e., no power, water leaks, roof leaks, no heat) contact Operations & Services at 978-934-2601, 8 a.m. to 5 p.m., Monday through Friday, after 5 p.m. contact University Police at 978-934-2394.
