Invitation to Self-Identify

Equal Opportunity and Outreach introduced the Invitation to Self-Identify form (pdf file) in 1998 and has updated it according to changing regulations.  This form is sent to all applicants of University benefited positions.  The applicants are asked to self-disclose their race, sex, and veteran/reserve status, and whether or not their age is 40 years or more.  Equal Opportunity and Outreach may refer to this information in its review of equity in hiring.

For faculty positions: So that we may mail this survey to applicants, Equal Opportunity and Outreach asks that you email us the names and addresses of all applicants.  Please do so using the Faculty Applicant Tracking Chart, described at Forms.  Please call your associate with any questions.
