Forms

There are three types of forms for inventors described here:

  1. Confidentiality Disclosure Agreement (CDA)
  2. Material Transfer Agreement (MTA)
  3. Invention Disclosure Form

Confidentiality agreements, also called non-disclosure agreements (or "NDA"), are often an essential part of an early exchange of information. The University offers three types -- a bilateral (most common), one way in (where the other party discloses to us but we disclose nothing to them) and one way out (where we disclose to the other party but they disclose nothing to us). It is important to describe the field in the document.

Please note that if you are planning to communicate any confidential information to outside parties, it is required for you to send a confidential disclosure agreement to them.  You are to notify and communicate what was disclosed to CVIP.  The Confidential Disclosure forms listed below are used as a reference guide only, any revisions must be approved and signed by Vice Chancellor Louise Griffin, on behalf of the University.

Material Transfer Agreements (PDF)

Material transfer agreements (or "MTA") are used when there is a transfer of a tangible item from the University to an outside party and fro an outside party to the University; this applies to both research and industrial uses of the materials.  A typical example would be the provision of a monoclonal antibody to an outside organization in support of their research. 

Any revisions to the standard MTA must be approved by Vice Chancellor Griffin.  MTAs are signed by Vice-Chancellor Louise Griffin, on behalf of the University.
