Lab Reservation Form

The Department of Physical Therapy has lab space available in Weed 304, 306, 310, 312, and 316.  These labs are available to faculty and DPT students. EP students should request use of the lab thru a faculty member.

Laboratory availability should be confirmed before submitting your request.  Calendar schedules can be found by opening Microsoft Outlook, clicking on the Public Folders icon and viewing the Physical Therapy Labs calendars.

Laboratory reservations can be made by using the Lab Reservation form and entering the details.  It will be added to the calendar and a confirmation e-mail will be sent to you.  For additional information contact Ann_Bratton@uml.edu.

Students are required to follow all laboratory guidelines and procedures as posted in the labs.
