All club members must complete the online registration form or Informed Consent and Member Information (hard copy) and read, print, and save the Massachusetts Hazing Law.

• Sport Club Renewal Application Form
• SC Member Information (complete with waiver form)
• Informed Consent for Minors: must be signed by parent or guardian
• Informed Consent for Student
• SC Roster Form
• Travel Approval Form: (Pre-Travel) submit at least one week in advance of the date your club wishes to travel
• Travel Reimbursement Form: (Pre-Travel) turn in with Travel Approval Form if expecting to use club funds
• Post Activity Report: (Post-Travel) submit within 48 hours after (1) return from travel, or (2) hosting an event
• Budget Request Form
