Human Subjects Forms

Submit all forms (as Word documents) and correspondence to IRB@uml.edu.

The PI Assurance and Signature Page may be faxed to x6012, scanned and emailed to IRB@uml.edu, or intercampus mailed to IRB Office, 2nd Floor Wannalancit.

Note: Once you are assigned a protocol number, please refer to it on all correspondence.

Send all IRB application submissions and correspondence to IRB@uml.edu. If your application size exceeds the capacity for one email (i.e., too many attachments), please submit via multiple emails, and number them accordingly (1 of 3, 2 of 3, etc.).
