Promoting Healthy Behaviors for High-Risk Elders

The University of Massachusetts Lowell School of Health and Environment and the Visiting Nurse Association of Greater Lowell are working together to offer a new Healthy Elder Living Program—HELP.  The primary goal of HELP is to promote healthy behaviors for home-dwelling individuals at high risk of re-hospitalization following their discharge from home health care.
