The Healthy Elder Living Program—H.E.L.P. includes an individualized program for each client developed by a geriatric specialist in conjunction with the specific discharge plan developed by the Lowell VNA.  The individualized program will be implemented in your home by trained students.  The students will visit with you twice monthly for a six month period. 

Should you decide to participate in the program, an initial screening to evaluate your health problems will be performed by a geriatric specialist.  The screening will be repeated after six months of participation in the program.  After one year, a follow-up telephone interview will be conducted to determine if you have been re-admitted to the hospital or home care services. 

You can decide to discontinue your participation at any time during the program.

It is our hope that the health and well-being of high elderly adults can be maintained over an extended period of time by providing occasional home visits to reinforce healthy behaviors and promote adherence to medical recommendations.
