Information Technology is beginning to develop new solutions for administrative offices to collect electronic forms from end users. There are a couple different technologies available with each having different functionality. The goal is to improve efficiency and service to internal and external customers.
GT Forms for PeopleSoft
The University of Massachusetts system has engaged Gideon Taylor as a partner to deliver robust eforms solutions within our existing PeopleSoft enterprise applications. This vendor has extensive experience work within the higher education industry to provide workflow driven eforms business solutions. The Lowell campus has purchased a limited number of licenses to use Adobe's EchoSign solution to securely capture signed documents via the internet. These forms can be securely sent to a person via email or can be completed online via a secure web page. Human Resources is using this to collect personal data from new employees. Human Resources eForms for New Employees
The following is meant to demonstrate the capabilities of the "eforms for the web" solution which is being implemented. It is NOT yet a production solution.
These forms can be completed directly from your home or office computer. It does require you to have access to Adobe Acrobat Reader and a working email address to validate your identity.
New Employees should complete and esign the following forms online once they have been informed they are being hired. These forms are automatically routed to Human Resources once they have been signed and email validated:
