All club members of an active club must complete the online individual member registration form  as well as the hard copy of the Informed Consent form.

• Sport Club Renewal Application Form
• Informed Consent for Minors
• Informed Consent for Student
• Sport Club  Roster Form
• Travel Approval Form: Must submit at least 1 week in advance of the date your club wishes to travel
• Travel Reimbursement Form: Turn in with Travel Approval Form if expecting to use club funds
• Post Activity Report: Submit within 48 hours after (a) return from travel, or (b) hosting an event
