The office of Institutional Research provides this form for handling and tracking data requests by internal and external requestors. Upon submission of this form, the requestor will receive an automatic acknowledgement. Shortly thereafter, a staff member will be in contact to discuss the request in more detail, including approximate scheduling of deliverables.
