
Health Insurance
Since September 1989, Massachusetts State Law has required all college students to be enrolled in a student health insurance program that meets required benefits established by the state. Those benefits, subject to change, are summarized as:
- Comprehensive coverage of primary and preventative care, emergency services, surgical services, hospitalization benefits, ambulatory patient services, and mental health benefits.
- $50,000 maximum aggregate indemnity per illness or injury per year.
- Coverage 24 hours a day, anywhere in the world, for a 12-month period with some exclusions, co-payments and deductibles.

PLEASE NOTE - This Waiver process MUST be completed by the specified deadline date of EACH academic year, even if it was completed the year before.
MASSHEALTH COVERAGE
All students selecting to waive the University Student Health Insurance Plan, stating that they have MassHealth coverage, are required to submit official documentation to the Student Health Services Office, McGaurvran Hall, South Campus, which confirms that they have such coverage (i.e., original notice on MassHealth letterhead). If such documentation is not received by the deadline date indicated on the www.isis.uml.edu web site, the charge will be re-instated on your student bill.
COMMONWEALTH CONNECTOR
Please note that Massachusetts college students are not eligible to enroll in the new health insurance plans being offered by the State on July 1, 2007.
Note: Since March 2000, the Division of Healthcare Finance and Policy has had in effect the following regulations regarding managed care coverage.
Managed Care Plans: Any student who has a managed care plan and will be living outside of his/her coverage area while attending the University of Massachusetts Lowell, must choose a primary care provider in the Lowell area or elect to enroll in the University Student Health Insurance Plan. Often, lab work procedures and/or prescriptions from the Student Health Services Office at the University of Massachusetts Lowell will not be covered by a managed care plan and the student may be responsible for all expenses incurred.
INTERNATIONAL STUDENTS
All International students are required to be enrolled in the University Health Insurance Plan. This requirement is authorized under the insurance laws for higher education students in Massachusetts (section 3.05(2) or MGL 114.6.
STUDENTS' RESPONSIBILITIES
If students have comparable coverage, they may select to waive the University Health Insurance Plan.

- Be prepared to answer a series of questions.
- Log into www.isis.uml.edu.
- Follow the health insurance link.
- Click on NO (I'm already covered by a comparable health insurance plan elsewhere).
Your answers and insurance information are subject to an audit. If for some reason there is a question regarding your answers or insurance coverage, the charge will be re-instated to your student bill, and you will be asked to provide further information.

Who must complete the on-line student health insurance selection?
- ALL UNDERGRADUATE students enrolled in a degree-granting program, whether FULL-TIME or PART-TIME, will be charged for the University Student Health Insurance Plan and are required to complete the on-line selection process.
- ALL GRADUATE students and ALL CONTINUING EDUCATION students enrolled in a degree granting or certificate program carrying nine or more credit hours per semester will be charged for the University Student Health Insurance Plan and are required to complete the on-line selection process.
EACH student MUST complete either the YES (I want the University Student Health Insurance Plan) or NO (I'm already covered by a comparable health insurance plan elsewhere) by the deadline indicated on the web page.
Students who do not complete the waiver process will be enrolled in the University Student Health Insurance Plan, and the health insurance charge will remain on the student's account.
NOTE: Students requiring proof of full-time enrollment for their own private insurance must submit this request to the Registrar's Office, Dugan Hall, South Campus.

On-Campus Insurance
This is a mandatory supplementary accident insurance coverage charged to all students: Graduate, Undergraduates, Encore, full or part-time Matriculating and Non-Matriculating. This insurance guarantees coverage of medical costs incurred due to injuries on campus or while participating in a University sponsored activity. On-Campus Insurance picks up medical costs 100% up to $10,000 maximum for whatever balance is left after claims have been submitted to and paid for by the student's own insurance company. On-Campus Insurance covers all students with any insurance plan.

Student Union Fee
Funds derived from this fee are used to support the Student Union buildings.
Operating Fee
The Operating Fee is the primary fee to support general operations at the University of Massachusetts Lowell. This fee provides revenue for instructional activity such as laboratory supplies, equipment upgrades and adjunct faculty salaries; for academic support such as the library; for student services such as athletics, student activities, counseling and health services; facilities costs, including utilities, for financial aid, and for administrative services.
College/Department Specific Fees
Certain colleges and departments charge fees in order to provide resources for college/department specific expenses not supported by the operating fee such as supplies and equipment used in introductory and higher level courses, maintenance and upgrade of laboratories and other college facilities, software licenses and other curriculum related costs. The use of these fees is limited to support of the college/department from whose students the fee is collected.
First Year Student Services Fee
The First Year Student Service Fee is a fee to provide a strong first year experience that enhances retention for all students. The fee will be used to meet the critical need for orientation programs and ongoing student support services.
Nursing Exit & Specialty Exam Fee
In order to practice as a nurse following graduation, each nurse must pass a national nursing license exam. This exam is a rigorous test of nursing knowledge and skills and is used both to evaluate the individual student and to monitor the quality of nursing education.

Advanced Health Assessment Fee
The Advanced Health Assessment Fee is charged to Nurse practitioner graduate students enrolled in the course 33.651. These students are taught a variety of health assessment skills in hospital settings.
Tuition
Tuition rates are established annually by the Massachusetts Higher Education Coordinating Council.
International Student Fee
International students require special services and numerous visas and other immigration forms before and after they arrive on campus. This is used to defray the cost of the staff, equipment and consultants necessary to handle international student services.
Late Fee
A $50 charge per semester will be assessed to anyone who is currently enrolled, has an outstanding balance and does not have a legitimate exemption as of the due date on the invoice. Exemptions include participation in the Academic Management Services payment plan, sufficient outstanding financial aid and/or loans, or direct billing of an approved third party.
Returned Check Fee
A $30 returned check fee is assessed to cover the cost of checks returned to the University due to insufficient funds or closed accounts. Notification of returned checks will be sent by the Student Financial Services Office. A student who has issued two or more returned checks within any academic year or three returned checks over the course of his/her academic career will be required to pay any outstanding balances with cash, money order, bank check or validated credit card. Any graduating student who passes even one bad check prior to graduation exercises, will be required to pay his/her outstanding balance in the above noted methods.
Tuition Surcharge
Effective for the fall semester 1999-2000, the Board of Higher Education mandates a surcharge on Massachusetts state university and college students who take more than a required number of credits to graduate. The charge applies only to in-state and regional program undergraduates who began their studies at UML. The surcharge rate is equivalent to the out-of-state rate, per credit, for each credit in excess of 118% of "required credits" for the baccalaureate degree. "Required credits" shall include those for single and double majors, minors and credits required for formal certification or licensing. In addition, credits accumulated in the Division of Continuing Education courses will not be included in the count. Where appropriate, the surcharge will be itemized on the student invoice. Students are advised that an appeals process is available to them. Surcharge Appeal Petitions will be considered by a review committee.
The HOPE Tax Credit
This is a $1,500 per year nonrefundable credit for qualified higher education expenses incurred for at least half-time enrollment during the first two years of a student's academic program leading to a recognized educational credential at an eligible institution.
The Lifetime Learning Credit
This is a $1,000 per year nonrefundable credit for qualified higher education expenses incurred at any eligible institution.
